Sector Results for Wound Management Goals
The NSM LHIN has continued its commitment to evidence based care in wound management by asking organizations from three sectors (long-term care, acute care and home care) to report on key metrics of wound care. 
Acute Care - pressure ulcer prevalence, State II and greater, will be reduced by 15% from the identified baseline.

Home Care (CCAC) - Reduction in daily dressing change frequency to 15% of the total clients being seen for wound care management. Improvement in the use of compression in the treatment of venous stasis ulcers by 30% from baseline.

Long-Term Care - Pressure ulcer prevalence, State II and greater, will be reduced by 15% from the identified baseline.

Each organization, within their respective sector, is requested to submit their data reflecting their performance on their specific metric, quarterly, for reporting on this website.
These graphs illustrate the dedication of many clinicians across North Simcoe Muskoka, in providing their patients/residents/clients with evidence-informed and consistently applied, wound management care, which means better outcomes for the North Simcoe Muskoka residents.
