North Simcoe Muskoka Health-’e’ Tactical Plan 2009-2010

Ontario’s eHealth Strategy for 2009-2010 will deliver a comprehensive, person-focused, secure and private electronic system that will improve the way people receive care.  In support of that goal, eHealth Ontario (eHO) issued a call for proposals in August for projects to be funded and implemented over the next six to twelve months.

Toward an Integrated Regional Falls Program for Seniors in North Simcoe Muskoka Falls 

Unintentional falls are the leading cause of injury and accidental death among Canadians over age 65.   2004/2005 data showed that in Ontario a senior visited an Emergency Department every 10 minutes and was hospitalized every 30 minutes as the result of a fall.  Almost half of falls in seniors result in a minor injury and 5-25% lead to serious injury (i.e. fracture, sprain). Falls cause more than 90% of all hip fractures.  20% of seniors die within one year of a hip fracture.

Balance of Care Project

The North Simcoe Muskoka (NSM) Balance of Care (BoC) project was commissioned by the NSM Local Health Integration Network (LHIN) to inform its Supportive Living (SL) initiative.  The SL initiative aims to maintain older persons “at risk” of loss of independence safely and cost-effectively in the community.

Meals of Wheels Program

MOW

Over a two month period, Royal Victoria Hospital in collaboration with Barrie Red Cross Meals on Wheels (MOW) completed a needs assessment looking at the effectiveness of, interest in and requirements for a more comprehensive Meals on Wheels deliver model including identification and screening for nutritional risk both individually and community wide.
