Health Infrastructure Renewal Form

Hospitals are responsible for ensuring that their facilities are in a good state-of-repair by managing their capital assets, and planning renewal activities.

On a priority basis, the Health Infrastructure Renewal Fund (HIRF) provides funds to supplement a hospital's existing capital renewal program, and help address renewal needs (e.g., roofing systems, boilers, windows).

Under this program, hospitals may receive a grant for eligible projects, regardless of their own ability to raise a local share of the project costs. There is no cost-sharing requirement, and 100% of the grant may be used on a single project.

Hospitals may select projects based on their own priorities, subject to the program eligibility criteria (outlined in the guidelines, below).

Health System Improvement Proposal

The North Simcoe Muskoka LHIN works with multiple partners and networks to determine program priorities. These priorities are outlined in our three-year Integrated Health Service Plan and Annual Business Plan.

Health system improvements are the additions or enhancements to programs offered by health service providers to improve the access and care provided to health consumers. The H-SIP process has been developed collectively by Ontario's Local Health Integration Networks (LHINs) to facilitate and standardize the way health system improvement requests are submitted to the LHINs.

The H-SIP form is to be used by health service providers prior to submitting a business case to the North Simcoe Muskoka LHIN. The pre-proposal is designed to capture the intent and scope of plans for health system improvement.

This process utilizes the LHIN Priority Setting and Decision Making Framework – a common tool across all LHINs with consistent criteria to facilitate transparency and accountability in LHIN priority setting and decision making.  Please review the documents below prior to contacting the LHIN regarding funding requests.
