Tara Moffatt MN, RN, CHPCN (c), CON (c)Tara Moffat RN

Director, Regional Palliative Care Network of North Simcoe Muskoka

Tara started her nursing career in 2006, graduating with a Bachelor of Science in Nursing from Queen's University. She immediately focused her career in palliative care nursing and in 2009 obtained her Masters of Nursing with a focus on Leadership in Education and Health Policy, and a clinical focus of Hospice Palliative Care. Tara also maintains her national nursing certifications in palliative care and oncology. From her first shift as a Registered Nurse in 2006, Tara has had a passion for policy and system-level change that aims to improve the equitable access and delivery of quality palliative care to patients and families. Tara is an active member in the community, and her husband and three children enjoy the many outdoor adventures in North Simcoe Muskoka.

  


Rebecca Ellerson BSc, BScN, RN, CON(C), CHPCN(C)Dr. Kate McLachlin MD 

Clinical Co-Lead, Regional Palliative Care Network of North Simcoe Muskoka

Rebecca has been practicing palliative care nursing since graduating in 2008. She has served as the Clinical Program Manager at Algonquin Grace Residential Hospice in Huntsville for the past six years. Since January 2017, Rebecca has been the Multidisciplinary Clinical Co-Lead for the NSM Regional Palliative Care Network. . Additionally, she serves as the board chair for Enliven, an organization based in Muskoka that aims to provide proactive self-care services including programs, workshops, and events for people living with cancer and their caregivers.  She has a passion for quality improvement and ensuring that system-level change translates into high-quality palliative care outcomes for patients and their families.

  


Dr. Kate McLachlin MD, CCFP (PC) Rebecca Ellerton RN

Clinical Co-Lead, Regional Palliative Care Network of North Simcoe Muskoka

Dr. Kate McLachlin is a family physician practicing palliative care. She received her Bachelor of Health Science from the University of Western Ontario with an Honours Certificate in rural health. Kate completed her Doctor of Medicine and her specialty training in Rural Family Medicine at McMaster University, and has a Certificate of Added Competency in Palliative Medicine recognized by the Canadian College of Family Practice. In addition to her palliative care work, Kate maintains a full-time family practice in Collingwood and holds hospital privileges at the Collingwood General and Marine Hospital. She was previously the Medical Director at Campbell House Hospice in Collingwood. Kate is a busy mom to two children and enjoys all the activities South Georgian Bay has to offer.
